Where to find Patches in Elden Ring – Patches can be found in Murkwater Cave, a dungeon north of the Agheel Lake. Take the river up, and after the invasion from Bloody Finger Nerijus, enter the nearby cave. Make sure you have a torch before entering if you don’t have one already. 
Murkwater Cave itself is a very short dungeon. You’ll find bells which you can walk into which alert nearby enemies, but provided you aren’t surrounded, shouldn’t be too taxing to take down. When ready, head down the right path from the central room to find a fog wall. Inside is a treasure chest. Open the chest and Patches will appear in a ‘boss’ fight.

He wields a Short Spear and the Adjudicator’s Shield, Mephistopheles will reward you with a Colorless Demon’s Soul if you kill him for her. When killed, he will drop the Thief’s Ring, If you found Patches in The Tunnel City and he appeared in the Nexus, killing the Adjudicator will cause him to leave. You must find him in The Ritual Path and complete the event in order to have him return to the Nexus. Occasionally after having Patches return to the Nexus and become a vendor after the events in The Tunnel City and The Ritual Path, he can no longer be found there. If this happens you must talk to him in The Tunnel City or The Ritual Path at his usual location to get him to reappear once again in the Nexus. The world you can find him in is the one you didn’t first meet him in, i.e. if you first encountered him in The Tunnel City, then talk to him in The Ritual Path to trigger his reappearance. After your 9th or 10th playthrough and depending on the World Tendency, the explosion of the bug during the The Tunnel City event will kill Patches. To prevent this you must either avoid killing the bug by rolling through the wooden boxes to its left or quit the game immediately after killing it. Meaning: once the death blow has been dealt, and before the explosion. Once you reload your game, patches will still be there, although his “side quest” not finished. To finish the side quest, pick up the ring of flame resistance, then talk to Patches. Patches is the only merchant who can offer an unlimited supply of Heavy Arrows. It is very unwise to kill him or let him die in a game where you’re using bows frequently.
How do you know if you killed patches?
Patches is best kept alive until later in Elden Ring – 
Should you spare Patches, he will seemingly turn over a new leaf. He’ll offer some help to the player by opening up a shop. Much of what he sells are consumables, most notably Margit’s Shackle and a Stonesword Key, Other items include:

By joining the covenant there, Patches will give you a quest to assassinate Great Horned Tragoth. When this is done, Great Horned Tragoth will drop the Bull-Goat set that offers amazing poise stats. 
If players decide to kill Patches, no one will blame them. Patches has been causing trouble for players in many of FromSoftware’s games, including Elden Ring. However, his death is easier to overcome if players have no intentions of using the Bull-Goat set.

Patches’ most recent appearance was in Elden Ring, It came as no surprise that he was included in the game, but what was surprising was the nature in which he is discovered. Opening a chest in Murkwater Cave will trigger a boss fight with none other than Patches The Untethered,
- Predictably weak and easy, Patches will swiftly try to surrender when the player does a certain amount of damage.
- He will then open up a shop right there within the cave, but that isn’t the end of his story here.
- Patches can then be found within the lakes of Liurnia, where he attempts to trick the player into teleporting to a dangerous location.
He can also be found in Mt Gelnir, where he once again kicks the player off a cliff in an attempt to prevent them reaching Volcano Manor, before retreating to the Manor itself where the player can engage in a questline until he eventually becomes friendly.
